This Addendum is executed by ______________________________ (“Guardian”), who is guardian of the person only of the Tenant in the attached Lease and attachments. By executing below the Guardian states as follows:

1. Guardian provides consent for the Tenant’s residential placement contemplated by the attached Lease and attachments.

2. Guardian is not assuming any financial responsibility, on behalf of Guardian or Tenant, for the payment of rent or any other expenses or fees as required by the Lease and attachments. Guardian does not have authority or control over Tenant’s financial affairs.
